Additional documents based on your service
You may need to send extra documents if you plan to provide any of the following services:
- Ambulance services
- Care homes
- GPs or independent consulting doctors
- Home care (domiciliary care) agencies
- Online primary care
- Oral health
- Supported living services
If your service is not listed, you only need to send us the core documents with your application.
