During an assessment of Termination of pregnancy

Date of assessment: 12 September 2025. This was the first inspection of this service since it was registered with CQC in the spring 2024. Therefore, this was a fully comprehensive inspection, covering all five domains.

We found staff were very caring and kind and were passionate about working at the service. Learning from incidents was used to make improvements. Managers ensured there were enough staff with appropriate skills to keep women safe. The clinic was clean, and staff ensured women were treated in a safe environment. Staff provided care following up to date national guidance and obtained consent in line with best practice. Staff adapted appointments in order to meet the needs of women. Women could receive treatment at a day and time that suited them. Managers were appropriately trained and supported teamworking between staff. Staff felt able to raise concerns and stated action would be taken by senior managers. Managers used effective governance systems to manage and mitigate risk and maintain oversight of the service.

However, the signing in process at reception did not protect women’s confidentiality. The services numbers of women returning for repeat terminations was less than the national average, however, the figure remained high at 38%. There was no formal pathway for reviewing and supporting women having a repeat termination. There were leaflets for children’s immunization directly outside treatment rooms and the waiting area overlooked the children’s play area, which had the potential to be insensitive to women attending the service.

During an assessment of the hospital overall

NUPAS Aylesbury is managed by National Unplanned Pregnancy Advisory Service Limited, and provides a sexual health service including medical and surgical termination of pregnancies, sexually transmitted infection testing as well as implanting long-term contraceptives.

The service is run from a GP practice in Buckinghamshire, two days a week, on Tuesday and Friday, between 8am and 6pm. The service runs surgical lists on Tuesdays and appointments for medical termination and pre-surgery assessment on Fridays.

Women could receive treatment for a medical termination up to 9 weeks and 6 days, a surgical termination up to 13 weeks and 6 days. At the time of inspection, the service used local anaesthetic for surgical terminations but was moving to conscious sedation the following week. This would enable the service to extend the timeframe for surgical terminations up to 17 weeks and 6 days.

In the 12 months prior to inspection, the service completed 2,981 terminations, of those, 185 were surgical and 2,796 were medical terminations.
